Data breach notices submitted to our office in accordance with RCW 19.255 and RCW 42.56.590 are published in the table below for public education purposes. To read a notice, click on the name of the organization in the list.

You can submit a data breach notice to our office online through our Data Breach Notification Web Form. You can also find more information on submitting notices in the “Resources for Businesses and Agencies Affected by a Data Breach” section of our Data Breach Resources page.
